Community Nursing Care

Disability Support

• GENERAL NURSING ASSESSMENT: MONITORING OF BLOOD PRESSURE, PULSE, TEMPERATURE, BLOOD SUGAR LEVELS ETC
• POST-ACUTE CARE: WE HELP YOU TRANSITION FROM THE HOSPITAL TO YOUR HOME BY PROVIDING CARE AND SUPPORT YOU REQUIRE WHILE YOU RECUPERATE
• MEDICATION MANAGEMENT
• WOUND/STOMA MANAGEMENT
• CONTINENCE/CATHETER MANAGEMENT
• TRACHEOTOMY CARE ETC
• ASSISTANCE WITH HOME DAILY ACTIVITIES:
INCLUDING COOKING/MEAL PREPARATION, PERSONAL
HYGIENE, HOUSE CLEANING ETC
• COMMUNITY-BASED SUPPORTS: INCLUDING

Support Coordination

Specialist Disability Accommodation (SDA)

 

•  Support Connection:  This involves providing short-term assistance to implement your NDIS plan, identifying service providers and negotiating Service Agreements.

•  Support Coordination: This includes longer-term support to build your capacity to navigate the complexity of services and supports you need from a range of providers.

•  Specialist Support Coordination: This is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist Support Coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.

SDA refers to accommodation for those participants requiring specialist disability housing solutions. These accommodation help to assist with the delivery of supports catering for their extreme functional impairment and/or very high support needs. We can connect you with a purpose-built specialist disability accommodation that meets your needs and choice.
Respite/Short Term Accommodation
STA (Short term Accommodation) including Respite: We provide accommodation for participants needing to live out of home for a short period of time. Short term accommodation funding can be used for respite to support the participant and their careers. It can also allows the carer a short break from their carer’s role.
MTA (Medium term Accommodation): We can support participants needing a place to stay till their long-term housing is ready.
