Prohealth Community Care Services is a registered NDIS provider based in St Albans. We deliver in-home nursing, personal care, supported independent living and respite for participants across Victoria and southern New South Wales.

Information about funding, eligibility and support at home.
What is the NDIS?
The National Disability Insurance Scheme (NDIS) funds eligible people with disability to access support for everyday life, independence and community participation.
How can I check if I'm eligible for NDIS funding?
NDIS eligibility depends on age, residence and disability or early intervention requirements. The NDIA makes eligibility decisions. Check the current requirements on the official NDIS website rather than relying on a general summary.

Your NDIS plan sets out the funded supports you can use. These may include personal care, community participation and other disability-related support. Ask your planner or support coordinator how a particular service fits your plan.
What is supported independent living (SIL)?
SIL is help with daily tasks in your home, such as personal care, cooking and cleaning. It is support funding, not payment for the home itself. The level of help depends on your needs and NDIS plan.

We currently provide support across metropolitan and regional Victoria, Albury/Wodonga, and the Murrumbidgee area of NSW, including Wagga Wagga, Griffith and Leeton. Our head office is in St Albans, Melbourne.
How do I choose an NDIS provider?
Ask about the services you need, staff experience, availability in your area and how the provider will work with your goals. You can speak with Prohealth before deciding whether our support is right for you.
What is the difference between SDA and SIL?
Specialist disability accommodation (SDA) is purpose-built housing for people with extreme functional impairment or very high support needs. SIL is help with daily living in a home. They are separate types of funding.
